As with the location of your incision, you have options for the placement of your implant. But bear in mind, post-mastectomy breast reconstruction is not a simple procedure. There are often many options to consider as you and your doctor explore what’s best for you. This information will give you a basic understanding of the procedure – when it’s appropriate, how it’s done, and what results you can expect. The Best Candidates for Breast Reconstruction Most mastectomy patients are medically appropriate for reconstruction, many at the same time that the breast is removed. The best candidates, however, are women whose cancer, as far as can be determined, seems to have been eliminated by mastectomy. Still, there are legitimate reasons to wait. Many women aren’t comfortable weighing all the options while they’re struggling to cope with a diagnosis of cancer. Others simply don’t want to have any more surgery than is absolutely necessary. Some patients may be advised by their surgeons to wait, particularly if the breast is being rebuilt in a more complicated procedure using flaps of skin and underlying tissue. Women with other health conditions, such as obesity, high blood pressure, or smoking, may also be advised to wait. In any case, being informed of your reconstruction options before surgery can help you prepare for a mastectomy with a more positive outlook for the future. All Surgery Carries Some Uncertainty and Risk Virtually any woman who must lose her breast to cancer can have it rebuilt through reconstructive surgery. There are risks, however, associated with any surgery and specific complications associated with this procedure. Although they are relatively uncommon, some problems of surgery in general, such as bleeding, fluid collection, excessive scar tissue, or difficulties with anesthesia, can occur. As with any surgery, smokers should be advised that nicotine can delay healing, resulting in conspicuous scars and prolonged recovery. Occasionally, these complications are severe enough to require a second operation. If an implant is used, there is a remote possibility that an infection will develop, usually within the first two weeks following surgery. In some of these cases, the implant may need to be removed for several months until the infection clears. A new implant can later be inserted. The most common problem, capsular contracture, occurs if the scar or capsule around the implant begins to tighten. This squeezing of the soft implant can cause the breast to feel hard. Capsular contracture can be treated in several ways, and sometimes requires either removal or scoring of the scar tissue, or perhaps removal or replacement of the implant. Reconstruction has no known effect on the recurrence of disease in the breast, nor does it generally interfere with chemotherapy or radiation treatment, should cancer recur. Your surgeon may recommend continuation of periodic mammograms on both the reconstructed and the remaining normal breast. If your reconstruction involves an implant, be sure to go to a radiology center where technicians are experienced in the special techniques required to get a reliable x-ray of a breast reconstructed with an implant. Women who postpone reconstruction may go through a period of emotional readjustment. Just as it took time to get used to the loss of a breast, a woman may feel anxious and confused as she begins to think of the reconstructed breast as her own.

At some point in their life, patients will require either removal or replacement of the breast implants. The most common reasons patients request explanation are ruptured implants, capsular contractures, and the desire for elective removal of the implants. Why Consider Breast Implant Removal with Dr. Farrell? We are happy to evaluate and consult our patients who are seeking advice concerning the removal of their breast implants. It is important to determine the age, location, and type of implant used, as well as, any history of pregnancy, weight gain or loss, and breast size before augmentation. Patients should have a base line mammogram within 12 months and a breast examination prior to implant removal. It is important that we discuss with our patients the expectations for post explanation breast size and the nature of aging on the augmented breasts. Patients who choose not have to have breast implant replaced may desire additional contouring procedures such as breast lift or mastopexy to improve the breast shape and nipple position. We are interested in caring for those patients, who are interested in the removal of breast implants without replacement. Is there a Risk of Complications After Breast Implant Removal? Management of the breast following explantation, implant removal, and capsulectomy has improved over the last several years. The most common reasons patients request explantation are ruptured implants, severe or recurrent capsular contractures, and the desire for elective removal based upon continued concern about silicone gel-filled implants. Evaluation Before Breast Implant Removal It is important, when possible; to determine the age, location and type of implant that was used, as well as any history of pregnancy, weight gain or loss, and pre-augmentation breast size. In addition a baseline mammogram should be completed at least within 12 months prior to surgery to assess the overlying breast tissue for pathology. Examination of the breasts to evaluate the degree of preoperative ptosis, the amount of breast tissue overlying the implant, the areolar size, and the size and position of the implant and degree of skin elasticity. The severity of the pre-operative breast ptosis is the most important determinate as to whether a patient is a candidate for breast contouring after removal of the implant. The thickness of the residual breast tissue determines the safety of a simultaneous breast contouring· procedure with explantation. If a mastopexy or breast lift is desired skin elasticity the implant position and nipple /areolar size are important factors. Explantation The goal of explantation is to remove the implant and the capsule if possible without implant rupture. Residual capsules can affect mammograms which make breast evaluation more difficult. Another reason to remove the capsule is the presence of residual silicone particles within the capsule. With explantation drains are often placed at the time of surgery and are usually removed one week later. Breast Contouring Options After Explantation There are several options for patients desiring explantation. The patient can undergo breast contouring alone or breast contouring with implant replacement or explantation alone. The type of breast contouring usually performed is called a mastopexy, which involves a rearrangement of the nipple/areolar position and removal of excess skin. Some patients require a delay of breast contouring surgery due to risk factors related to paucity of breast soft tissue, smoking, and the need to transpose or move the nipple more than several inches. Removal of breast implants or explantation has become an important aspect of plastic surgery since in the last several decades over one million women have undergone breast implant insertion for a variety of procedures and an awareness of the issues related to breast implant removal is important to all women who currently have breast implants. Breast Implant Removal (Explantation) Implanted breast prostheses are made of silicone shells filled with silicone gel or saline. Implantation can be indicated for medically necessary reasons such as breast reconstruction following a mastectomy or for cosmetic reasons. Complications can occur with breast implants that would require removal of the implant. Examples of complications include infection, extrusion (implant exposure through the skin), rupture, and contracture. A contracture occurs when the capsule surrounding a breast implant tightens. The breast will feel hard and become painful. Disfigurement can result if the capsule around one implant contracts and the other does not. When male breast-reduction surgery is performed by a qualified plastic surgeon, complications are infrequent and usually minor. Nevertheless, as with any surgery, there are risks. These include infection, skin injury, excessive bleeding, adverse reaction to anesthesia, and excessive fluid loss or accumulation. The procedure may also result in noticeable scars, permanent pigment changes in the breast area, or slightly mismatched breasts or nipples. If asymmetry is significant, a second procedure may be performed to remove additional tissue. The temporary side effects of male breast reduction include loss of breast sensation or numbness, which may last up to a year. How to Prepare for Male Breast Reduction Surgery The initial consultation with your surgeon is very important. Your surgeon will need a complete medical history, so check your own records ahead of time and be ready to provide this information. First, your surgeon will examine your breasts and check for causes of the gynecomastia, such as impaired liver function, use of estrogen-containing medications, or anabolic steroids. If a medical problem is the suspected cause, you’ll be referred to an appropriate specialist. Your plastic surgeon may, in extreme cases, also recommend a mammogram, or breast x-ray. This will not only rule out the very small possibility of breast cancer, but will also reveal the breast’s composition. Once your surgeon knows how much fat and glandular tissue is contained within the breasts, he or she can choose a surgical approach to best suit your needs. Skin cancers are usually limited to the area of the skin lesion and the surrounding tissues. The most common types of skin cancer are basal cell carcinoma and squamous cell carcinoma. Melanoma is the type of skin cancer that may spread through the lymph nodes or the blood systems. Treatment of skin cancer involves the surgical removal of the cancerous lesion and the surrounding tissues with methods that preserve facial lines and appearance. For some types of skin cancer, reconstruction may require more than one procedure to achieve the best results. Skin cancer is the most common form of cancer in the United States. More than 500,000 new cases are reported each year, and the incidence is rising faster than any other type of cancer. While skin cancers can be found on any part of the body, about 80 percent appear on the face, head, or neck, where they can be disfiguring as well as dangerous. The primary cause of skin cancer is ultraviolet radiation, most often from the sun, but also from artificial sources like sunlamps and tanning booths. In fact, researchers believe that our quest for the perfect tan, an increase in outdoor activities, and perhaps the thinning of the earth’s protective ozone layer are behind the alarming rise we’re now seeing in skin cancers. Who Gets Skin Cancer…and Why? Anyone can get skin cancer, no matter what your skin type, race or age, no matter where you live or what you do. But your risk is greater if… Your skin is fair and freckles easily. You have light-colored hair and eyes. You have a large number of moles, or moles of unusual size or shape. You have a family history of skin cancer or a personal history of blistering sunburn. You spend a lot of time working or playing outdoors. You live closer to the equator, at a higher altitude, or in any place that gets intense, year-round sunshine. You received therapeutic radiation treatments for adolescent acne. Types of Skin Cancer By far the most common type of skin cancer is basal cell carcinoma. Fortunately, it’s also the least dangerous kind; it tends to grow slowly and rarely spreads beyond its original site. Though basal cell carcinoma is seldom life-threatening, if left untreated it can grow deep beneath the skin and into the underlying tissue and bone, causing serious damage (particularly if it’s located near the eye). Squamous cell carcinoma is the next most common kind of skin cancer, frequently appearing on the lips, face, or ears. It sometimes spreads to distant sites, including lymph nodes and internal organs. Squamous cell carcinoma can become life threatening if it’s not treated. A third form of skin cancer, malignant melanoma, is the least common, but its incidence is increasing rapidly, especially in the Sunbelt states. Malignant melanoma is the most dangerous type of skin cancer. If discovered early enough, it can be completely cured. If it’s not treated quickly, however, malignant melanoma may spread throughout the body and is often deadly. Other Skin Growths You Should Know About Two other common types of skin growths are moles and keratoses. Moles are clusters of heavily pigmented skin cells, either flat or raised above the skin surface. While most pose no danger, some-particularly large moles present at birth, or those with mottled colors and poorly defined borders-may develop into malignant melanoma. Moles are frequently removed for cosmetic reasons, or because they’re constantly irritated by clothing or jewelry (which can sometimes cause pre-cancerous changes). Solar or actinic keratoses are rough, red or brown, scaly patches on the skin. They are usually found on areas exposed to the sun, and sometimes develop into squamous cell cancer. Recognizing Skin Cancer Basal and squamous cell carcinomas can vary widely in appearance. The cancer may begin as small, white or pink nodule or bumps; it can be smooth and shiny, waxy, or pitted on the surface. It might also appear as a red spot that is rough, dry or scaly; or it might be a firm, red lump that may form a crust. It could look like a crusted group of nodules or like a white patch that resembles scar tissue. It may also present itself as a sore that bleeds or doesn’t heal after two to four weeks.
